The complete process of Panchakarma involves the following three steps: 

A. Poorva Karma

Preparatory procedure required before the main treatment

Benefits:

  • It facilitates softening the tissues so that the lipid-soluble toxins deposited into them gets liquefied and flow back into the digestive tract. From here, they can be eliminated.
  • This treatment prepares the patient mentally and physically for the main procedure of Panchakarma.

It involves three procedures:

  1. Pachan Karma – improves digestion with herbs and fasting so that the patient can digest the ghee (clarified butter) which is provided to liquefy the fat-soluble toxins.
  2. Snehana Karma – medicated ghee is given to the patient in increasing doses in order to aggravate and liquefy the fat-soluble toxins deposited in the deep tissues.
  3. Swedana Karma – a full body steam bath is given to the patient thereby opening up the body channels and allowing the heat to liquefy the toxins further. This facilitates their movement from the tissues to the digestive tract.

B. Pradhan Karma

This is the Panchakarma, a five-step procedure which is highly individualized depending on the needs, age, digestive strength, immune system, and other factors. This intense Panchakarma procedure can only be done under the guidance of an Ayurvedic practitioner. The five karmas to cleanse the complete body are:

1.  Vamana (therapeutic emesis) –
It includes induced vomiting which helps clear the upper GI tract to the duodenum (end of the stomach) and a part of the respiratory tract.
Benefits:
Acts on Kapha buildup in the chest, upper gastrointestinal tract and the head for this reason it is an excellent therapy for individuals with Kapha constitution and in Kapha related disorders.

2. Virechana (purgation) – Virechana, therapeutic diarrhea, is done after Vamana to cleanse denser toxins that are of sticky Kapha and acidic Pitta in nature.
Benefits:
The body draws out these endotoxins from the liver, gallbladder, spleen and tries to eliminate them through the small intestines then from the duodenum to the exit.
A good virechana brings about a feeling of lightness and ease in the body.
Taste and appetite also gets increased.
3. Basti/ Vasti (Enema): Decoction or Sneha is administered through rectal route

Benefits:
Is half the treatment in itself, Acharya Charaka calls it the ‘greatest and the most effective’ treatment of all.

The colon, urinary bladder and urethra are irrigated, nourished and flushed clean with herbal decoctions and oil extracts.

Highly effective procedure in Diseases caused due to Vata or patients who have health concerns of Vata origin

There are three kinds of Vasti:

  • Anuvaasana / Maatra Vasti or oil based enema is given to those with severe dryness, who have high vata and who have a powerful digestive Agni. This is a snehana oleation therapy.
  • Aasthapana / Niruha Vasti is an enema with herbal drugs extracted in liquid.
  • Uttara Vasti is enema of the urethra and urinary bladder.A successful Vasti creates a sense of well being in the body, the body feels light and a feeling of great relief is felt. Appetite and digestion improves.

4. Nasya – It is the nasal administration of medicated substances that involves instillation of medicated powder, juice, or herbal oils through the nostrils.
